Submitting Life Claims Just Got Easier!

  • The original Certified Death Certificate (CDC) is no longer required
  • Dearborn National will now accept a PHOTOCOPY of the CDC

For life claims with a total coverage amount of $25,000 or less

Features Function
Modify Search Saves your last search option and refreshes the dropdown menus when you make your new selections.
Add Button Lets you add a form to your Package.
Remove Button Lets you delete a form from your Package.

Packages the new forms into one PDF file and allows you to e-mail or print the PDF by clicking on the appropriate icons.

Note: Required forms are highlighted in yellow and are automatically added to the "Package" section on the right.
