Filing a Claim: Accelerated Life Benefit

What is an Accelerated Life Benefit?
Accelerated Life Benefit pays a lump sum benefit if you or your insured dependent is diagnosed with a terminal condition and has a life expectancy of twelve months or less. Group life benefits are reduced by the amount of the Accelerated Life Benefit payment.

What is the deadline for filing an Accelerated Life Benefit claim?
There is no time limit for filing this type of claim.

How do I apply for an Accelerated Life Benefit?
A completed Accelerated Life Benefit claim form is required. All claim forms require information to be provided by three parties: the insured employee/retiree, the benefits coordinator or ERS, and the attending practitioner. You must complete identifying information in the Employee's Statement and forward the form to your benefits coordinator (if you are an employee) or ERS (if you are a retiree) and they will complete the Employer's Statement and return the form to you. When you receive the form, have your doctor complete the Attending Practitioner's Statement. Then, complete, sign, and date the claim form and forward with all attachments to Fort Dearborn Life at the address shown on the claim form.

What is the time frame to process an Accelerated Life Benefit check?
The time frame to process an Accelerated Life Claim check for an ERS Group Life claim is based on information that is received with the claim. If enough medical information has been received to support the request, then the check would be processed within 10 calendar days if approved by Dearborn National.
