The current Benefits Book and Brochure for Active Employees describes the Disability Insurance benefits available to active employees for Plan Year 2012, effective September 1, 2011.
- The Benefits Book provides detail on your Disability Insurance benefits coverages available, as well as step-by-step instructions for filing claims.
- The Benefits Brochure gives a high-level outline of the Disability benefits available.
In the event of a conflict or inconsistency between the provisions of the benefits book and the master group policy or plan document, the master group policy or plan document will control.
